School Nurse

ABSENCE:  All absences must be reported by 8am via our online absence form.

ABSENCE FORM

ILLNESS at home:  If a student is sick, the student should remain home for the health and safety of all other students and teachers.

  • If seen by a physician for medical treatment, please bring documentation for the student's medical file. 
  • If a student is being treated with medications for any contagious disease, the student should be medicated a minimum of 24 hours prior to returning to school (i.e. strep throat).  
  • If a student has a fever of 100.4 degrees or greater, he/she should remain home and "fever free" for 24 hours (without medication such as Advil/Tylenol) before returning to school. 
  • If the student has diarrhea or is vomiting, he/she can return to school 24 hours after the last occurrence of vomiting or diarrhea.  
  • If a student accrues more than 10 absences in a school year, a doctor’s note is required for each subsequent absence.

ILLNESS AT SCHOOL:  Students may be sent home at the discretion of the school nurse or school official. Students will be sent home if a student has a fever of 100.4 degrees or greater and s/he should remain home and "fever free" for 24 hours (without medication such as Advil/Tylenol) before returning to school.  If a student vomits or has diarrhea they must go home and cannot return to school until 24 hours after the last occurrence of vomiting or diarrhea. A parent or emergency contact must be available to pick up the student within 30 minutes. Please remember that staff are following the directives of our medical advisor and recommended practice. It is important that you pick up your child in a prompt timeframe. We appreciate your cooperation in advance.  

 

 

MEDICATION:  As a parent or guardian, it is your responsibility to alert the health office if your child requires medication. Parents/guardians are responsible for dropping off their child's medication (i.e. EpiPens, inhalers, etc) at the start of each school year and replacing this medication upon expiration. These medications must be accompanied by doctor's orders which must be signed by both the doctor and parent/guardian (see form under "links to Common Health Forms).

Failure to have unexpired medication and current orders will result in your child not participating in certain school activities, but not limited to, field trips, athletics, etc. To avoid this, please set a reminder on your calendar or phone to ensure that your child's daily and/or emergency medication and orders are current.

PHYSICALS:  The Connecticut General Statutes (Section 10-204a and Section 10-206) require that each child attending school has an up-to-date health assessment (physical) on file and be adequately immunized. 

New students will require a physical and compliant immunization records.  Out of state forms will be accepted with a doctor's signature, clearly indicating a student is cleared to participate fully in the program/school and physical activity.

All students entering Kindergarten & Grade 7 for the 25-26 school year must show proof of the mandated health assessment/immunizations, dated on or after July 1, 2024, in order to start school before the first day of the school year.  See link to the right for copies of the health form.

SPORTS PHYSICALS: any student participating in a school sponsored sport (i.e. basketball/track) must have a current physical on file with the school nurse before he/she can participate in that sport. Sports physicals are valid for 13 months from the date of the physical (use the age appropriate Health Assesment Record Form).

 

SCREENINGS: The following screenings are required by the State of Connecticut and will take place in the health office with the school nurse:

Vision:  grades K,1,3,4,5

Hearing: grades K,1,3,4,5

Scoliosis: GIRLS in grades 5 & 7                                               BOYS in grade 8

Parents will be advised via emails of dates these screenings will be taking place, and may notify the health office if they do not want the screening performed.  The screenings will not be done if already performed by their PCP dating on or after 7/1/25.  

PRE-SCHOOL

FLU VACCINE: The State of Connecticut mandates that all students in Pre-School receive the flu vaccine by December 31.  Students who do not have a flu-shot on record with the school nurse before Christmas break will not be allowed to return to school after Christmas break until proof of this immunization is received by the school nurse.
